Overview

The State of Vermont is looking for an experienced Social Worker and/or Nursing Home Administrator to take their career to the next level and hope that the Department of Disabilities, Aging and Independent Living (DAIL)-Division of Licensing and Protection is that place. We are hiring social worker and/or nursing home administrator who will work as healthcare surveyor to ensure that our facilities are following the rules and regulations set forth by the State of Vermont and Federal Government (Centers for Medicare/Medicaid-CMS). As a social worker and/or nursing home administrator you have the potential to bring valuable experience, knowledge and perspective to our teams. You will be working with a team of Registered Nurses with varied backgrounds, who have come from hospitals, home health, long term care, public health, and health care administration.


Although this is a home-based job, it requires a significant amount of travel throughout the state of Vermont, and you must be able to spend nights in a hotel several times a week/month. The work week is typically three days out in the field with the fourth day spent working in your home office. There will be occasional off-work expected, in the evenings and weekends, and you must also have your own reliable means of transportation.


By using your knowledge, objectivity, professionalism and commitment, the Health Facility Surveyor is expected to:

  • Assess the quality of care and services of health care facilities/providers via onsite observations, interviews and record reviews. Identify any deficient practices and relate those findings to federal and state statutory and regulatory requirements. The facilities/providers we cover include Nursing Homes, Acute Care Hospitals, Critical Access Hospitals, Psychiatric Hospitals, End Stage Renal Dialysis Centers, Ambulatory Surgical Centers, Rural Health Clinics, Home Health Agencies and Hospice Agencies.
  • Collaborate with other surveyors to evaluate compliance in providing quality care, quality of life, and other regulatory requirements. Each team member participates in evaluating a heath care facility/provider's compliance in providing quality care, quality of life and other structural requirements.
  • Write deficiency statements according to the Centers for Medicaid and Medicare (CMS) "Principles of Documentation".
  • Complete reports and summaries according to guidelines and timelines set forth by CMS and the state.
  • Demonstrate proficiency in utilizing computer programs and applications.

Within twelve months of hire, the successful candidate will be required to pass a Centers for Medicare and Medicaid (CMS) Federal Qualification Test (SMQT).

Duties are performed in both field and office settings. Considerable travel to facility locations is required. Evening and weekend work is required. Overnight stays in a hotel are an expected requirement on a monthly basis. Resistance or objection to standards or to survey findings may be anticipated and requires considerable tact and diplomacy to achieve appropriate resolution. Survey deadlines, exit conferences, and interviewing may occasion considerable stress.

Minimum Qualifications

Possession of, or eligibility for, licensure as a social worker (LICSW or LMSW) or nursing home administrator (LNHA) in Vermont OR eligible to practice in the state of Vermont via a multi-state license AND four (4) years or more of professional social worker or nursing home administrator experience.

NOTE: Surveyor Minimum Qualifications Test (SMQT) certification is required within one year of hire.

NOTE: Must maintain Vermont licensure or multi-state license as a social worker or nursing home administrator as a condition of employment.

Preferred Qualifications

Bachelor’s Degree.

Candidates must pass any level of background investigation applicable to the position. In accordance with AHS Policy 4.02, Hiring Standards, Vermont and/or national criminal record checks, as well as DMV and adult and child abuse registry checks, as appropriate to the position under recruitment, will be conducted on candidates, with the exception of those who are current classified state employees seeking transfer, promotion or demotion into an AHS classified position or are persons exercising re-employment (RIF) rights.
