The ideal candidate will work collaboratively within our healthcare teams to ensure that our patients receive personalized and comprehensive care tailored to their specific needs during their recovery processes. This role includes developing care plans, overseeing the execution of treatment plans, and acting as a liaison between patients, their families, and healthcare providers.

Responsibilities

• Assess new patients, determining appropriate levels of care and specific patient needs.
• Develop comprehensive and personalized care plans, in coordination with other healthcare professionals.
• Regularly review and update patient plans as per their recovery progress.
• Act as the primary point of contact for families and patients, providing timely updates and information.
• Ensure compliance with healthcare regulations and standards in patient care management.
• Facilitate patient access to required medical and support services, scheduling appointments and transportation as necessary.
• Strong interpersonal and communication skills.
• Ability to handle complex situations and make decisions under pressure.
• Proficiency in medical software and case management systems.
• Strong organization and time management skills.
• Knowledge of healthcare regulations and policies.
