Qualifications

• At least 2 years of customer service experience, preferably in health care.
• Strong problem-solving, critical-thinking and organizational skills.
• Ability to prioritize, organize, plan and manage multiple tasks.

Responsibilities

• Serve as member liaison throughout program life cycle with support and resources.
• Communicate with members and caregivers to uncover barriers to outcomes and reduce avoidable visits.
• Complete member welcome calls on notification or discharge.
• Manage appointment scheduling, confirmations and reminders; mail letters as needed.
• Identify and connect members to resources addressing social determinants of health (SDOH).
• Document all phone calls and interactions accurately in the system.
