Job summary

Bonaventure Senior Living in Keizer, OR invites applicants for a Health and Wellness Coordinator to support daily health services operations and onboarding of residents.

You will coordinate care with internal teams and external providers, maintain compliance and documentation, and lead ongoing health-services training. This role emphasizes resident-centered care and effective communication with families and leadership.

Qualifications

• Have or obtain state certifications and licenses.
• Demonstrated experience supervising team members and providing training and support.
• Understand and follow State regulations as well company policies and guidelines.
• English language required.
• CPR Certification (preferred).
• Must pass criminal background check and drug test.

Responsibilities

• Welcome new residents and help provide a smooth, supportive onboarding experience.
• Participate in daily stand-up meetings and communicate resident concerns, incidents, and other operational issues with community leadership.
• Complete regular quality assurance, operational, and resident-chart audits for Assisted Living and Memory Care.
• Review occurrence and incident reports, coordinate appropriate interventions, and assist with required regulatory reporting.
• Maintain required health services documentation, including state-specific assessment tools, service plans, progress notes, and resident records.
• Coordinate service plan updates, provider recommendations, medication revisions, nursing assessments, and other changes in condition.
• Manage care coordination with outside healthcare providers, including admissions, care plans, appointments, documentation, and ongoing communication.
• Maintain regular communication with residents’ families and facilitate family meetings to ensure each resident’s needs, preferences, and goals are being met.
• Communicate residents’ dietary needs, allergies, exceptions, and preferences to the appropriate team members, and keep information current.
• Lead ongoing Health Services training, track required staff credentials and testing, and present monthly wellness education for residents.
• Participate in resident retention efforts, scheduled community meetings, and the rotating Manager on Duty schedule, including occasional after-hours responsibilities.
• Perform additional duties as assigned to support residents, team members, and community operations.
