Overview
We are hiring an
Outreach Specialist on behalf of our healthcare client in Los Angeles to support community outreach and participant enrollment for the PACE (Program of All-Inclusive Care for the Elderly) program. This role is ideal for individuals passionate about elder care and experienced in healthcare, Medicare, or health plan sales.
The Outreach Specialist will work closely with local community partners, prospective participants, and caregivers to educate and raise awareness about the PACE program, helping frail older adults remain independent in their homes.
Key Responsibilities
- Build and maintain strong relationships with community organizations and healthcare providers.
- Promote awareness of the PACE program through outreach efforts and public events.
- Educate individuals, families, and caregivers on program benefits and eligibility.
- Collaborate with Enrollment Specialists to support seamless onboarding of participants.
- Organize and attend local events, senior center visits, and health fairs.
- Maintain accurate activity and contact records using CRM tools.
- Ensure compliance with DHCS and CMS outreach standards.
- Support development and execution of enrollment marketing plans.
- Assist in census growth and retention initiatives.
Qualifications
Required:- Associate degree or higher.
- 1+ year of experience in any of the following:
- Enrollment Sales
- Healthcare Sales
- Health Plan Sales
- Medicare Sales
- Must live within 10 miles of Downtown Los Angeles (zip code 90057).
- Valid CA driver’s license, personal transportation, clean driving record, and auto insurance.
- Familiarity with the LA County healthcare landscape.
- Bilingual in English and Spanish, Korean, or Tagalog.
- Must have spoken with a Relode recruiter via phone regarding this role.
Preferred- 1+ year of experience in:
- Value-Based Care (VBC)
- PACE (Program of All-Inclusive Care for the Elderly)
- Background in senior care or field-based outreach (e.g., VA centers, senior centers, SNFs)
Work Environment
- Hybrid position: 2–3 days/week in the field and/or clinic center.
- Orientation sessions held the 1st and 3rd week of every month.
- Interview process duration: Approximately 2–3 weeks.
