Position Summary

The Health Navigator will work within our Federally Qualified Health Centers (FQHCs) to support patients in accessing and navigating healthcare services with a focus on the Medication to Address Opioid Use Disorders (MOUD) program and closing essential gaps in care. This role is essential in improving health outcomes by providing education, resources, and guidance for underserved populations, including adults with Serious Mental Illness, uninsured, underinsured, and vulnerable community members. The Health Navigator collaborates with healthcare providers, case managers, and external community resources to promote patient-centered care and facilitate access to essential health services. The Health Navigator will provide BHT level behavioral health and skills interventions to facilitate overall wellness and health to patients served. This position reports to the Health Center Director or Clinical Site Manager.

Qualifications

• High School Diploma or GED in combination with one (1) year of behavioral health experience.
• Full‑time availability. Monday‑Friday daytime shift, with flexible availability for weekends & evenings.

Duties

• Patient Education, Advocacy & Outreach: serve as a trusted patient advocate, support patients in understanding care plans, provide health education and promote health literacy, disseminate health materials.
• Care Coordination & Navigation: assist patients with appointment scheduling, referrals, and follow‑up care; accompany patients to appointments; coordinate with care management teams; provide ongoing support to patients engaged in care management; follow up on missed appointments and reengage patients.
• Community Resource Connection: connect patients to SDOH resources; maintain knowledge of community-based resources; develop and maintain community collaborations; participate in outreach at community events and educational activities, including provider health fairs.
• Communication & Follow‑Up: follow up on patient phone calls; coordinate timely, team‑based responses; ensure patients are informed, supported, and empowered throughout their care journey.
