Job summary

LVMC Clinic is seeking a Health Information Technician to manage day-to-day HIM functions across LVMC Clinic locations in California. Responsibilities include importing documents into eCW, organizing paper records for scanning, and processing release of information requests while maintaining confidentiality.

Position Summary:



  • The Clinic Health Information Technician is responsible for the day-to-day Health Information Dept. functions at LVMC Clinic locations. HIM clinic functions may include the following tasks and activities:

    • Import requested health information documents from Altera into eClinicalWorks for patient care at the clinic

    • Organizes and prepares paper documents for scanning into the electronic health record (EHR).

    • Scan and validate paper documents received via electronic fax or from clinic staff into patient medical records in eClinicalWorks (eCW)

    • Respond to patient, family, and payor basic (non-legal) release of health information requests.

    • Health information technician may help other clinic sites as needed.



  • This position reports to the Health Information Management (HIM) Dept. Lead.

Position Duties/Responsibility:


  • Must be able to use all applications on multiple software platforms. These include but are not limited to eCW, Affinity, Sunrise Clinical Manager, Quick Chart, Fluency for Transcription and other ancillary medical records repositories.

  • Evaluate, investigate and complete merge requests in eCW.

  • Must be able to evaluate validity of requests for release of medical records from patients and/or family members and process these requests when a valid authorization has been received.

  • Must have basic Federal and State laws for release of health information for medical records.

  • Must be able to produce medical record copies in paper and electronic form.

  • Maintains confidentiality of the medical records accessed in performance of job duties.

  • Must have the ability to use interpersonal skills to interact effectively with physicians, co-workers, and other customers of the medical records department. Skills include being able to interact with professionalism, courtesy and respect at all times.

  • Must have the ability to organize and prioritize work tasks effectively with little supervision.

  • Must be able to revalidate scanned documents at the rate of 3 pages per minute.

  • Maintains adequate supplies for the clinic HIM area, ordering supplies as needed.

  • Helps at other Clinic locations with release of information requests as needed in the event of absence of other location staff.

  • Provide support at other LVMC locations as needed to ensure adequate staffing and maintain efficient operations.

  • Other Duties Assigned by Dept. Lead and/or Director


Essential Functions:


  • Personal computer skills to manage multiple systems and screens for performing work functions.

  • Strong attention to detail to ensure the accuracy of data and information, and to detect any inconsistencies or anomalies in medical records.

  • Technical ability to utilize electronic health record system and various scanning hardware and software and to understand impact links to the medical records.

  • Understand and comply with the laws and regulations related to health information, including the Health Insurance Portability and Accountability Act (HIPAA).

  • Ability to maintain confidentiality and adhere to ethical standards for the handling of sensitive patient information.

  • Must have excellent reading, writing, spelling and grammar skills and understand and follow oral/written instructions as needed.

  • Must have excellent communication skills with the ability to communicate clearly and effectively in person, in writing and on the phone.

  • The ability to use the keyboard to do data entry.

  • Perform standard Windows system functions.

  • The ability to sit for long periods of time.

  • The ability to work as a positive Team member.

  • The ability to have positive personal interactions with staff, patients and visitors.

  • Must be able to meet performance competencies.


Position Qualifications:


  • Education: High School diploma or equivalent required. Associate Degree in Health Information Management recommended, but not required.

  • Experience: Computer and other modern office equipment knowledge. Health Information Management department experience preferred.

  • C ertifications: None required.

  • Skills/Ability: Knowledge of modern office equipment. Excellent verbal, grammar, spelling, and written communication skills. Exhibits the ability to organize work assignments and follow through with accuracy, exercise good judgment, demonstrates initiative, tact and poise. Must have excellent verbal and written communication skills to interact with healthcare providers, patients and visitors.
