Job summary

Monarch Healthcare Management is seeking a Health Information Manager to oversee the health information system and medical records for residents, ensuring accuracy, confidentiality, and compliance across the facility. The HIM leads documentation, audits, and coding to support quality care and regulatory readiness.

The role emphasizes safeguarding PHI, coordinating release of information, maintaining census data, and supervising HI staff while collaborating with leadership on QAPI initiatives.

Qualifications

• Previous HI Assistant or HUC experience; training in medical records.
• Knowledge of regulations, survey process, documentation requirements and legal issues.
• Understanding of payment systems including Medicare and Medicaid
• High School graduate or equivalent
• Knowledge of medical terminology
• Long-term care or healthcare experience

Responsibilities

• Health Information Security & HIPAA Compliance: Safeguard confidentiality, integrity, and security of medical records per HIPAA and regulations.
• Release of Information & Record Retention: Manage requests and maintain compliant filing, retention, and destruction processes.
• Census Management & Data Integrity: Maintain daily census data, case mix changes, pay status updates with audit trails.
• Admission & Ongoing Records Management: Coordinate admissions, initiate and maintain medical records, perform audits and coding.
• Discharge Record Completion & Quality Review: Oversee discharge documentation and ensure timely physician completion.
• Department Leadership, QAPI & Collaboration: Supervise HI staff and support HIPAA compliance and resident rights
