Role Description

This is a part-time hybrid role for a Health Home Plus Care Manager located in NYC area. The Care Manager will be responsible for coordinating and providing care management services to individuals with serious mental illness in the Health Home Plus program. The role involves conducting assessments, developing care plans, coordinating with healthcare providers, and ensuring clients receive appropriate services. Some work from home is acceptable.

Qualifications

• Case Management, Care Coordination and behavioral health knowledge
• Strong communication and interpersonal skills
• Ability to work independently and in a team setting
• Experience with mental health populations
• Bachelor's degree in Social Work, Psychology, Public Health, or related field
• Previous experience in behavioral health or social services is a plus
• Ability to work with individuals from diverse backgrounds and presentations
