Job summary

A leading healthcare organization is seeking a Case Manager for the Pawtucket Health Home Team in Providence, RI. This full-time role involves coordinating services for consumers to reduce psychiatric and substance abuse disorder symptoms while advocating for their needs. Candidates should have an Associate's degree (Bachelor's preferred) and experience in the human services field. A valid driver's license and insured vehicle are required. The position also offers a $2500 retention bonus.

Qualifications

• Experience in Human Services field preferred.

Responsibilities

• Coordinate a variety of services for consumers.
• Provide support counseling and problem solving.
• Teach consumers adult daily living skills.
• Complete recordkeeping in accordance with center requirements.
