Job Title: Health Home Care Coordinator

Join Ohel Children's Home and Family Services as a Health Home Care Coordinator.

Description

Ohel is seeking a Care Coordinator to manage the care of children enrolled in its Health Home program. Responsibilities include assessing physical, mental health, and social service needs, developing integrated care plans, collaborating with healthcare and social service providers, and providing care coordination, health promotion, transitional care, follow-up, and family support. The role requires use of health information technology and involves monthly home visits. Candidates must have a Bachelor's Degree and at least 2 years of experience in case management or working with children with behavioral health needs. This is a full-time position based in Brooklyn.

Salary: Bachelor’s Level $50,000; Master’s Level $55,000.
