Job summary

Qualifications

• Active RN license in the state of residence.
• 3+ years of clinical experience in hospital, acute care, home health/hospice, direct care or care management.
• Strong presentation and coaching abilities.
• Proficiency with MS Word, PowerPoint and Excel.

Responsibilities

• Educate members on health education and condition management.
• Refer employees to clinical programs.
• Create and manage employer-specific programs.
• Analyze health data and develop targeted programs.
• Interface with facilities and providers for issue resolution.
• Know community resources for referral and outreach.
• Travel up to 25% (out-of-state).
