Physical Requirements

This position will require driving within a designated service area to provide linkage navigation and outreach services to persons accessing comprehensive medical service in partnership with the Lake Cumberland District Health Department.

Shift

This position must be willing to work in different environments (both office-based and on the ground), at times best suited to the community (including some evenings and weekends). This includes hours outside of traditional office hours (i.e., 8:00 a.m. – 5:00 p.m. or 9:00 a.m. – 6:00 p.m.).

Job Summary

The work location for this position is: Lake Cumberland District Health Department

The Health Education Coordinator will provide staffing for Harm Reduction programs within the Lake Cumberland District Health Department.

Staff will be contracted UK employees that will work solely for the Lake Cumberland District Health Department to:

• Deliver health education, prevention counseling, and risk-reduction services related to HIV , Hepatitis C, and STIs.
• Assess client needs and risk factors; provide referrals to medical, psychosocial, PrEP, harm-reduction, and supportive services.
• Promote health literacy, care retention, and treatment adherence through counseling, education, and distribution of prevention supplies.
