THIS POSITION IS REMOTE/WORK FROM HOME IN THE POSTING STATES.

THE POSITION STRONGLY PREFERS A REGISTERED DIETITIAN LICENSURE.

WORK SCHEDULE: Monday - Thursday 8AM - 4:30PM or 8:30AM - 5PM; 2 evening shifts 10:30AM - 7PM per week; 1 last shift on Friday/month 9:30AM - 6PM (Central Time Zone hours).

Position Purpose:

Assist members by completing health assessments and implementing effective solutions and care plans to improve their health and wellness. Work with members at risk or diagnosed with chronic conditions to provide support, encouragement, and education.

1. Utilize clinical training and knowledge to support members with chronic health conditions.
2. Perform telephonic assessments to evaluate needs, clinical risk, and readiness to change.
3. Provide telephonic support and coaching on wellness care plans and programs to improve health and lifestyle through outreach and education.
4. Help members develop attainable wellness goals with plans, support, encouragement, and resources.
5. Monitor wellness plans and member outcomes, and recommend adjustments to reduce health risks.
6. Engage members in decisions related to maintaining a healthy lifestyle.
7. Coordinate with healthcare providers to align wellness and medical care plans.
8. Maintain member information in compliance with standards.
9. Provide feedback to leadership for quality improvement.
10. Perform other duties as assigned and ensure policy compliance.

Education/Experience:

Associate's degree and 2-4 years of related experience required.

License/Certification:

• Registered Dietitian, Physical Therapist, Respiratory Therapist, RN, Master's in Behavioral Health, or Health Coach Certification required.

Pay Range: $26.50 - $47.59 per hour

Actual pay will be adjusted based on skills, experience, education, and other factors. Total compensation may include additional incentives.
