Join to apply for the Health Coach - Wellness role at Centene Corporation.

This position is remote/work from home in the posting states.

Prefers a Registered Dietitian licensure.

Work schedule: Monday - Thursday 8AM - 4:30PM or 8:30AM - 5PM; two evening shifts 10:30AM - 7PM per week; one shift on Friday or Monday 9:30AM - 6PM (Central Time).

Position Purpose

Assists members by completing health assessments and implementing care plans to improve health and wellness. Supports members with chronic conditions through education, encouragement, and support.

Responsibilities

1. Utilize clinical training to support members with chronic health conditions.
2. Perform telephonic assessments to evaluate needs and risks.
3. Provide telephonic coaching on wellness care plans and programs.
4. Help members develop attainable wellness goals.
5. Monitor wellness plans and outcomes, adjusting care as needed.
6. Engage members in maintaining a healthy lifestyle.
7. Coordinate with healthcare providers to align wellness and medical plans.
8. Maintain member information for compliance.
9. Provide feedback to improve care quality.
10. Perform other duties as assigned.

Education & Experience

Associate's degree and 2-4 years of related experience required.

Licenses & Certifications

• Registered Dietitian, Physical Therapist, Respiratory Therapist, RN, Master's in Behavioral Health, or Health Coach Certification required.

Pay range: $26.50 - $47.59/hour.
