Job summary

Saint Thomas Community Health Center in New Orleans seeks a Health Coach to partner with patients and the medical team to promote healthier lives. The role focuses on care plans, chronic care management enrollment, and supporting access to care in a community clinic.

The ideal candidate is compassionate, a good listener, and skilled at problem solving, with a bachelor’s degree welcomed and a preference for Kinesiology or Health Science, working on-site in New Orleans.

Qualifications

• Compassionate, good listener with problem-solving skills.
• Bachelor degree welcomed; preference for Kinesiology or Health Science.
• Experience or comfort with chronic care management is a plus.

Responsibilities

• Meet with patients to promote health goals.
• Review social determinants of health and care plans.
• Enroll patients into Chronic Care Management.
• Discuss care plans by phone and in person.
• Utilize Motivational Interviewing principles.
• Document in EPIC EHR and coordinate with nurses and providers.

The role includes developing relationships with patients, reviewing care plans, and setting goals. The health coach offers supportive guidance to help patients foster healthier lives.
