Job summary

Carillon House Health Center is seeking an Admissions Coordinator to manage the admissions process and maintain open communication with referral sources, physicians, and payers. This role requires coordination of referrals, interviews, and accurate documentation prior to admission.

The ideal candidate has knowledge of Medicare Part A, is experienced in long-term care, and possesses strong written, verbal, and interpersonal skills to juggle multiple priorities in a fast-paced environment.

Qualifications

• Good working knowledge of Medicare Part A and certification procedures.
• Experience in a long-term care setting desired.
• Excellent written, oral and interpersonal skills, as well as the ability to juggle multiple priorities.

Responsibilities

• Obtains referrals from hospital social workers, physicians, or family members, keeping wait lists current.
• Conducts admission interviews with resident or responsible party, collecting financial data, signatures, medical information and physician details prior to admission.
• Reviews admission paperwork including Medicare Part A coverage and non-coverage procedures; maintains admissions files per resident.
• Maintains census activity: admissions, discharges, deaths, transfers, and payment method changes.
• Keeps an ongoing daily/monthly census of residents in the Health Center; tracks admissions and transfers.
• Coordinates utilization review for new admissions covered by Medicare Part A.
• Maintains confidentiality of admission information and resident records.
• Assists in coordinating admissions/transfers/discharges with other departments.
• Informs resident, family, and responsible party of resident's rights.
