Job summary

Bellin Health is seeking a Health Care Records Representative in Green Bay, WI to maintain patient records, scan documents into Epic, and support timely access to information for care continuity.

In this full-time role, you will collect paperwork, enter data, and communicate with care teams while upholding confidentiality and quality standards. Hybrid work eligibility may apply after 6 months.

Qualifications

• High School Diploma or equivalent.
• Strong communication skills and attention to detail.
• Prior healthcare background, preferred but not required.
• Epic knowledge is preferred but not required.
• Comfortable working independently.
• Ability to maintain confidentiality.
• Valid Driver's License, Required.

Responsibilities

• Reviews patient information from external facilities and enters information into the electronic health care record for providers and patients to view, in an accurate and timely manner.
• Effectively communicates with providers and support staff on health information processes.
• Rounding to hospital and clinic departments, gathering documentation and scanning into the document imaging systems.
• Generates and customizes excel spreadsheets, along with storing completed work in designated locations.
• Maintains knowledge of parts of the chart for process related to paper medical records.
• Maintains productivity and quality metrics as defined by management.
• Distributes communication notices to patients and providers in an accurate and timely manner.
• Interprets and identifies appropriate information for health care record disclosures based on applicable State and Federal laws and/or regulations and according to organization and departmental policies and procedures. This includes the copying and disclosure of Protected Health Information (PHI) contained in the electronic and paper patient health care record.

Description

Emplify Health is seeking a dedicated Health Care Records Representative to join our team! You will play a crucial role in maintaining accurate and accessible patient medical records.

Day-to-day you will be responsible for a variety of duties that support the timely collection, distribution, analysis, completion, and availability of the patient health care records to assist with continuity of care while maintaining quality standards of data.

You will collect paperwork from various inpatient units and scan these patient documents into Epic; such paperwork could entail consent forms, admission paperwork, transfers from the ER, and other medical records.

Job Specifics:

Location:1920 Libal St, Green Bay, WI 54301 | Hybrid schedule eligibility after 6 months
Work Schedule:Monday - Friday. Business hours. No weekends or holidays
