Job description

This position is located in Edmond or Guthrie, Oklahoma.

Health Care Management Nurse

Annual Salary:

Level I: - Y15A - $58,055.13 per year + full state benefits package

Level II: - Y15B - $62,409.27 per year + full state benefits package

Travel is extensive - Must possess a valid driver's license and maintain required car insurance. Occasional short-notice home visits are required, and the candidate must be able to report within an acceptable timeframe.

Work Hours: Monday - Friday, 8:30 am - 5:00 pm initially.

Minimum Qualifications:

Level I:

  • Possession of a valid permanent Oklahoma registered nurse license as approved by the Oklahoma Board of Nursing or via the Registered Nurse Licensure Compact (eNLC).
  • Two years of professional nursing experience.

Level II:

  • Possession of a valid permanent Oklahoma registered nurse license as approved by the Oklahoma Board of Nursing or via the Registered Nurse Licensure Compact (eNLC).
  • Three years of professional nursing experience.

This Registered Nurse position involves conducting medical eligibility assessments for Title 19 nursing home admissions, State Plan Personal Care, and ADvantage Waiver applications. It requires a thorough evaluation of medical care eligibility for each applicant.

Job Responsibilities:
  • Conduct telephone, electronic, and/or in-person home visits within the assigned area to complete comprehensive assessments, identify needs, and make medical eligibility recommendations for long-term care programs. Reliable internet connectivity is essential for remote work.
  • Must have stable and reliable home internet (not a hotspot); in-office work may be required.
  • Ability to frequently lift up to 15 lbs. for transporting equipment.
  • Assist in other areas as needed due to high application volume.
  • Assist with Adult Protective Services evaluations upon request.
Knowledge, Skills, and Abilities (KSAs):
  • Ability to analyze complex health issues, evaluate adherence to healthcare standards, and assess the effectiveness of health management plans, recommending changes as needed.
  • Work independently with a high degree of professionalism, possibly leading special projects. Must have professional nursing knowledge to review and analyze healthcare services to ensure appropriate resource utilization.
