Posting Summary

Rutgers University seeks a Health Care Case Manager in Essex Central for the Child Health Program within the School of Nursing, located in East Orange, NJ. The HCCM supports the Department of Children and Families in meeting healthcare needs of children in out-of-home placement, collaborating with the Child Health Unit.

Key duties include:

• Assisting with enrollment and medical home identification for children entering out-of-home placement.
• Assessing healthcare status by reviewing medical records and interviewing providers and caregivers.
• Scheduling and reviewing initial physical health evaluations.
• Developing child healthcare plans with medical providers and caregivers.
• Connecting children and caregivers with health resources and services.
• Ensuring timely healthcare visits and forwarding reports to medical providers.

Position Details

Qualifications

• Bachelor's Degree in Nursing or a health-related field; experience may substitute for degree.
• 1-2 years pediatric nursing experience required.
• Current RN licensure in NJ and BLS certification required.

Preferred: 2+ years in nursing case management or community health nursing.
