Rutgers, The State University of New Jersey, is seeking a Health Care Case Manager for Essex Central for the Child Health Program within the School of Nursing, located in East Orange, NJ. The Health Care Case Manager (HCCM) functions to support the Department of Children and Families (DCF)/Child Protection and Permanency (CP&P) in meeting the health care and protection needs of children in out-of-home placement, in collaboration with other staff of the Child Health Unit in that local office.
