The Fund for Public Health in New York City (FPHNYC) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers. To this end, in partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNYC incubates innovative public health initiatives implemented by DOHMH to advance community health throughout the city. Itfacilitates partnerships, often new and unconventional, between government and the private sector to develop, test, and launch new initiatives. These collaborations speed the execution of demonstration projects, effect expansion of successful pilot programs, and support rapid implementation to meet the public health needs of individuals, families, and communities across New York City.

Program Overview

The Viral Hepatitis Program (VHP) at the NYC DOHMH’s Bureau of Hepatitis, HIV and STIs, works to reduce morbidity and mortality associated with Hepatitis B and C in NYC. The VHP operates a Telephone Navigation Program to support New Yorkers in accessing hepatitis B and C care. The Telephone Navigation Program aims to support outreach, linkage to hepatitis care and supportive services, health education, care coordination, and hepatitis treatment completion.

This position will be funded through OD2A, via a five-year cooperative agreement with the Centers for Disease Control. This position is housed at the New York City Department of Health and Mental Hygiene in Long Island City.

Position Overview

The Health Care Access Specialist position will be part of the VHP Telephone Navigation Program to support the VHP’s telephone navigation and surveillance investigation activities, including but not limited to outreach, patient interview, provider interview, linkage to hepatitis care and supportive services, supports in hepatitis treatment initiation and completion process, care coordination, health education on hepatitis B/C and drug use safety, referral intake, data collection, data documentation, and presentation.

Responsibilities

The Health Care Access Specialist will reach out to and provide patient navigation services to people living with hepatitis B and/or C, hepatitis C and HIV co-infection, justice-involvement, history of substance use, and those tested positive for hepatitis B and/or C at a community-based facility but were not treated for hepatitis B and/or C.

  • Call patients and conduct patient interviews for surveillance investigation and linkage to care.
  • Assess needs and barriers to accessing hepatitis B/C care, confirm hepatitis B/C infection status, make medical appointments, provide reminders, and follow up with patients.
  • Provide hepatitis B, hepatitis C, liver health, and overdose prevention education to patients.
  • Refer patients to hepatitis care, supportive services, such as harm reduction, medication-assisted treatment, rehab, detox, health insurance enrollment, housing, and/or mental health.
  • Call providers for care coordination and medical records inquiries.
  • Perform referral intake and data collection.
  • Document outreach activities and medical records in the NYC DOHMH’s electronic disease surveillance system and maintain patient confidentiality.
  • Attend internal staff meetings and external meetings.
  • Give case presentation and participate in group discussion.
  • Deliver project presentation in meetings or conferences.
  • Support project planning, implementation, data reporting, evaluation, quality assurance, and material development.
  • Develop and update program materials.
  • Report to supervisor and leadership on the progress of projects.
  • Complete other tasks as assigned.

Qualifications

  • Bachelor’s degree from an accredited college or university with a specialization in public health, community health education, human services, social work, or a related field.
  • Basic knowledge of viral hepatitis testing, transmission prevention, and treatment is preferred.
  • Multilingual is preferred, such as fluent in French, Fulani, Russian, Spanish, or Wolof.
  • At least one year of experience in a similar role, working in a public health or health care setting.
  • Experience in linkage to medical care and providing health education by phone.
  • Experience in conducting surveys, assessments, or questionnaires with patients.
  • Comfort with talking about chronic disease, substance use, and mental health issues with patients.
  • Excellent problem-solving skills and ability to develop health care access plan with patients.
  • Excellent interpersonal skills and work well with diverse populations.
  • Strong oral, written, documentation, and presentation skills.
  • Strong time management skills and able to handle multiple navigation cases.
  • Highly organized with attention to detail.
  • Proficiency with Microsoft Word, Excel, PowerPoint, Outlook, and Teams.
  • Interest in working with vulnerable populations on hepatitis care, harm reduction and overdose prevention.

Work Schedule:

9:00 am – 5:00 pm, Monday to Friday

There is potential for a hybrid work schedule to be determined with the program.

This is a grant-funded position and will continue through August 31, 2028, contingent on the continuation of federal funding.

RESIDENCY REQUIREMENT

You must live in the New York City Tri-state area (NY, NJ, CT) to be considered for a position at FPHNYC.
