• Discharge planning and community/home programming adjustments.
• Conduct screenings and follow-up evaluations as needed.
• Communicate evaluation findings to patients, families, and team members.
• Supervise students and volunteers per state regulations.
• Document assessments, treatments, and billing times per department procedures.
• Ensure infection control and patient confidentiality standards are followed.
