Job summary

All County Home Health & Hospice in the Greater San Antonio area seeks a licensed Physical Therapist to provide rehabilitation services in home health settings. You will assess patients, implement plans of care, and coordinate with families and clinicians to optimize outcomes.

The role emphasizes delivering high-quality PT, educating patients and caregivers, and contributing to discharge planning while maintaining CPR certification and good communication.

Qualifications

• Requires degree in physical therapy from an accredited program.
• Licensed to practice as a physical therapist in the state.
• Two years of relevant PT experience preferred, home health a plus.
• Good verbal and written communication and organization skills.
• Must maintain CPR certification.
• Must have a valid driving license with insured vehicle.

Responsibilities

• Provide PT services per physician’s plan of care.
• Assess therapeutic status and contribute to total care plan.
• Evaluate strength, mobility, gait, and ROM for rehab potential.
• Direct PT treatment and educate patients and caregivers.
• Identify needs for other home health services and refer as needed.
• Prepare progress summaries and participate in discharge planning.
• Supervise assistants per policy and state regulations.
• Instruct and train patients and families in therapy use and goals.
