Job summary

All County Home Health & Hospice is seeking a licensed Physical Therapist to provide skilled services in the Greater San Antonio area. The PT will evaluate patients, develop and implement plans of care, and educate families and caregivers on therapy techniques and equipment use.

Responsibilities include supervising aides as required, coordinating with physicians, and contributing to discharge planning. A valid Texas PT license, CPR, and a driver’s license with a reliable vehicle are required.

Qualifications

• Degree in physical therapy approved by an accredited organization.
• Licensed to practice as physical therapist in Texas.
• Minimum two years of physical therapy experience; community/home health experience preferred.
• Current CPR certification.
• Must have reliable vehicle and valid driver's license.

Responsibilities

• Provide physical therapy services per physician's plan of care.
• Assess rehabilitation status and development of the care plan.
• Evaluate patient mobility, strength, ROM and rehabilitation potential.
• Instruct patients and families in use and care of therapeutic devices.
• Report patient progress to physician and adjust care as needed.
• Supervise/PTA per policy and state regulations.
• Discharge planning involvement and progress summaries.

This may include, but will not be limited to:Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. The incumbents may be requested to perform job related tasks other than those stated in this description.Position QualificationsPossesses a degree from a baccalaureate or master's program in physical therapy approved by an accredited organization.Licensed to practice as physical therapist within the State, if applicable.Two years of appropriate experience as a physical therapist.
