Acquire4Hire seeks a dedicated social services professional in New Hampshire to implement family services, health, and community engagement content at the assigned center.

The role involves building partnerships with families, coaching goal development, and coordinating with community resources to support family plans. Evening visits and occasional weekend activities may be required, with comprehensive benefits and a strong emphasis on client-centered care.

Qualifications

• Degree in Social Work, Psychology, Sociology or Human Services.
• Experience in case work or working with community organizations and small groups.
• Flexibility for evening home visits, meetings, and possible weekend activities.

Responsibilities

What You'll Do

Responsibilities Include, but are not limited to:

• Building positive and strength based partnerships with parents, within professional boundaries, coaching families in the process of identifying family development goals.
• Supporting families in creating and implementing family plans.
• Completing at least 3 visits per year with assigned families focusing on family goals, identification of needs and connecting to community resources.
• Providing ongoing family support and case management with assigned families to address identified needs, crises, referrals and follow up

Requirements

The candidate will have a degree in Social Work, Psychology, Sociology or Human Services; Prior experience in case work, community organizations, and working with small groups; Ability to be flexible with time for evening home visits, meetings, and possible weekend parent activities
