Job summary

Qualifications

• Must have at least 1 year of experience in children's health care.
• Bilingual English/Spanish is preferred.

Responsibilities

• Collects medical and developmental history for enrollment and maintains health charts.
• Performs health screenings, tracks immunizations, and coordinates referrals.
• Coordinates with contracted nurse and staff for health plans and follow-ups.
• Transports children to appointments as needed and maintains confidentiality.
