Job summary

Union County Community Action, Inc. (UCCA) seeks a compassionate, organized Case Manager to connect families with community resources, support children’s health and school readiness, and help families achieve their goals through Head Start services.

Work Location: Monroe, NC. Schedule: Mon–Fri 7:30am–4:30pm (40 hours). Starting Rate: $19.58 per hour. CDL required within 90 days and valid driver’s license. CDL/government benefits.

Qualifications

• Bachelor's Degree in Social Work, Sociology, Psychology, Human Services, or a related field.
• 1–3 years of related experience preferred.
• Experience in case management, family services, or outreach preferred.
• CDL required within 90 days of employment.
• Valid driver's license required.
• Bilingual preferred.

Responsibilities

• Recruit and enroll eligible children and families into the Head Start program.
• Develop and maintain Family Partnership Agreements with enrolled families.
• Connect families with community resources and support services.
• Assist families in scheduling and accessing health and developmental services.
• Conduct home visits and provide ongoing family support.
• Support parent engagement activities, meetings, and workshops.
• Maintain accurate records and data entry within ChildPlus.
• Collaborate with site staff and community partners to coordinate services.
• Assist with program recruitment and outreach efforts.
• Support transportation services as assigned.
