Job summary

AbsoluteCARE Medical Center & Pharmacy is seeking a strategic Director of Utilization Management and Complex Care Management to lead transitional care and complex care teams. Qualifications

• RN licensure in good standing and eligible to supervise relevant staff
• CCM credentials or equivalent certifications preferred
• 10+ years in healthcare leadership with experience in value-based care
• Strong data analytics, EHR proficiency, and performance improvement track record

Responsibilities

• Lead integrated care teams to reduce cost of care and admissions/1000.
• Use data analytics to monitor performance and drive improvements.
• Establish collaborative relationships with hospitals, SNFs, home health agencies, and community partners.
• Ensure adherence to privacy, HIPAA, and regulatory standards.
• Oversee staff development, onboarding, and performance coaching.
