The ND Department of Health & Human Services (HHS) Adult & Aging Services Division is seeking a Home and Community Based Services Case Manager to assist individuals in remaining in their own homes with supportive services. This is a Telecommute position serving Dickinson, Watford City, and nearby areas.
As our HCBS Case Manager, you will provide case management to elderly and physically disabled individuals under the Home and Community Based Services Program. Responsibilities include performing assessments, developing and implementing individualized person-centered care plans, monitoring, and ongoing case management. You will also make referrals to community partners and maintain records and documentation.
To succeed in this role, proficiency in computer use, good time management, and organizational skills are essential. The position requires visiting clients in their homes or facility settings and involves travel for on-site assessments.
Qualifications include licensure as a Licensed Baccalaureate Social Worker (LBSW) by the ND Board of Social Work Examiners or licensure as a Baccalaureate of Nursing with RN (BSN/RN) licensure by the North Dakota Board of Nursing. Additionally, one year of professional human services work experience or a Master’s Degree in social work with licensure as a Licensed Social Worker is required. Consideration may be given to applicants awaiting licensure exam results or with less than one year of experience if they meet other criteria.
Location: This role offers the flexibility to work from home with travel to Dickinson, Watford City, and surrounding areas.
