Position Title: Harbor Project: Medical Case Manager
Reports to: Case Management Coordinator

Status: Full Time, Non-Exempt, Eligible for benefits
Qualifications:
- Bachelor’s degree in social work, psychology, or related field required; Addiction Counseling Certification a plus. Master’s degree preferred; experience in social/human service delivery, mental health, and addiction counseling.
- Demonstrated expertise in case management, supportive housing, and client advocacy.
- Ability to work with diverse populations, including clients and staff, in critical and emergency situations.
- Sensitivity to cultural, economic, and other differences. Patience, creativity, flexibility, and compassion for people with disabilities.
- Knowledge of local resources and service systems. Understanding of medical and psychosocial issues related to HIV, mental health, substance abuse, and other conditions.
- Flexibility in work sites and hours, including occasional evenings and weekends. Valid driver’s license required.
Basic Function:
- Provide comprehensive HIV case management services to 20 clients with intensive needs.
- Collaborate with the Direct Services team to ensure high-quality services.
- Assist formerly homeless individuals living with HIV/AIDS, psychiatric disabilities, and/or substance abuse.
Specific Responsibilities:
- Complete intake assessments following Pathways procedures.
- Participate in on-call rotation.
- Perform face-to-face needs assessments, including medical, demographic, and consent information.
- Develop service care plans with clients and support team, maintaining confidentiality.
- Manage caseloads, providing case management and advocacy for clients with HIV, substance abuse, and mental health histories.
- Conduct bi-monthly home visits and accompany clients to medical and treatment appointments as needed.
- Outstationed at Pathways to Housing Inc.
- Assist clients in accessing social services and community resources.
- Provide information, referrals, follow-up, and reassessment.
- Educate clients’ support systems as appropriate, with informed consent.
- Deliver tailored, client-driven services such as housing placement, independent living skills, vocational guidance, and substance use treatment.
- Coordinate with care providers on discharge and home care plans.
- Consult with Action Wellness and partner staff regarding client plans.
- Participate in activities outside regular hours as needed.
- Provide intake coverage as assigned.
Performance Standards:
- Maintain knowledge of resources, benefits, and entitlements for client referrals and advocacy.
- Complete required reports.
- Provide leadership and backup coverage for team members.
- Coordinate with outside collaborators to facilitate service access.
- Adhere to clinical and procedural policies.
- Attend staff meetings, team rounds, and case conferences regularly.
- Maintain accurate, timely records and documentation of client contacts.
