Dungarvin in Vancouver, WA is seeking a Habilitative Program Administrator to join the Washington State Clinical Team. You will create and oversee habilitative plans for adults with IDD in the Supported Living Program and develop functional assessments and PBSPs.
You will train team members, partner with program directors, review data in Therap, track deadlines, and maintain communication with guardians and state partners.
The Habilitative Program Administrator (HPA) will serve as a member of the Washington State Clinical Team, creating and overseeing habilitative plans for adults with intellectual/developmental disabilities in the Supported Living Program in Vancouver
Use information and data collected through direct observation and meetings with individuals supported to develop Functional Assessments and Positive Behavior Support Plans (PBSPs)
Develop Individual Support Plan Programs for individuals on caseload
Train team members on plans and program implementation
Partner with program directors to implement support plan programs
Using electronic data collection system (Therap), review data and compile reports for other members of support team on behavior and habilitative goal progress
Assist in the creation and maintenance of tracking spreadsheets for relevant due dates and person-centered planning meetings
Keep up to date on all relevant state policy changes
Maintain open lines of communication with stakeholders outside the organization, i.e. guardians and DDCS partners
Attend support planning meetings as required
