Qualifications

• Minimum of one year experience working with individuals with disabilities required.
• Valid driver’s license, reliable transportation, and proof of insurance required.

Responsibilities

• Coordinate and implement habilitation services.
• Develop and maintain Individual Support Plans (PCISP).
• Facilitate transdisciplinary team meetings.

Skills

Strong verbal and written communication skills

Ability to develop reports and individualized support plans

Knowledge of person‑centered planning principles

Computer proficiency

Organizational and problem‑solving skills

Education

High school diploma or equivalent

Bachelor’s degree in a behavioral science field (preferred)
