Job summary

Blue River Services, Inc. seeks a Direct Support Professional – Habilitation to provide person-centered support to older adults and individuals with disabilities across program, community, and transportation settings.

You will implement PCISP plans, assist with daily living skills, health and safety needs, and community involvement while maintaining a safe, respectful environment. This full-time, non-exempt role follows a Monday–Friday schedule from 8:00am to 3:30pm.

Qualifications

• High school diploma preferred.
• At least one year of experience working with individuals with disabilities preferred.
• Commitment to confidentiality, safety and person-centered services.

Responsibilities

• Deliver direct support based on PCISP goals and abilities.
• Create age-appropriate activities to encourage participation and community inclusion.
• Assist with personal care, health, safety, and mobility needs.
• Support skill development in communication, self-care and social interaction.
• Maintain documentation of progress, services and incidents.
• Transport and accompany persons served to activities and appointments as needed.
