Job summary

ADAPT Community Network in New York, NY, is hiring a Habilitation Assistant to support individuals in achieving independence through daily activities and individualized plans. You will assist with personal care, implement skill-building, promote inclusion, and help maintain a safe, home-like environment.

Qualifications

• High School Diploma or GED required.
• AMAP certification required by hire or within six months.
• Valid New York State driver’s license.
• Ability to read, write, speak and understand English.
• Strong interpersonal and communication skills.
• Ability to respond to crisis and make decisions under pressure.
• Commitment to confidentiality and safeguarding rights.
• Ability to work independently and as part of a team.
• Passion for ADAPT’s mission and disability services.

Responsibilities

• Provide direct support and personal care with daily living activities.
• Assist with individualized program plans and behavior support strategies.
• Promote independence and community participation through self-help and social skills.
• Supervise classroom activities, community outings, and recreational programs.
• Ensure safety and positive engagement during all activities.
• Transport individuals to sites and activities using agency vehicles.
• Collect data and document progress on goals and plans.
• Prepare concise progress notes and incident reports.
• Participate in interdisciplinary team meetings and trainings.
• Maintain a clean, organized, and safe program environment.
• Assist with recreational activities, including swimming or gym sessions.
• Dispense medications after AMAP training under RN supervision.
• Respond to crises and medical emergencies as needed.
• Ensure respectful interactions and prevent abuse/neglect.
• Substitute in other program areas as needed.
• Perform other duties as assigned.
