Responsibilities

• Keeps up-to-date evaluation documentation compliant with Federal, State, and Local regulations.
• Develops psychosocial history and assessment including guest problems and strengths.
• Orients Guests and families to facility, services, rights and limitations.
• Coordinates discharge planning in collaboration with Guests and families.
• Maintains filing system for the Social Services Department and reports issues.
• Participates in interdisciplinary conferences and QA/P initiative activities.
