Job summary

Legacy Healthcare LLC in Chicago seeks a Social Worker to ensure the Guests' medically related emotional and social needs are met, in accordance with federal, state, and local regulations.

The role includes maintaining evaluation documentation, developing psychosocial assessments, coordinating admissions and transfers, and providing counseling and resource linkage to support individualized care plans. Collaboration with families and staff is essential.

Qualifications

• High School diploma or equivalent.
• Two years' experience in a social services role, preferably in a health care setting.
• Bachelor's degree in social work or similar field.
• 5 years related experience.

Responsibilities

• Maintain up-to-date evaluation documentation compliant with Federal, State, and Local regulations.
• Develop psychosocial assessments including problems, strengths, and cultural considerations for care plans.
• Coordinate admissions, transfers, and discharge planning within the facility.
• Provide informal counseling and connect Guests with community resources as needed.
• Participate in QAPI activities and mandatory compliance trainings.
