Responsibilities

• Screen calls and emails to determine eligibility and priority.
• Assess client needs and develop plans to meet those needs.
• Monitor client funds and prepare annual accounting reports.
• Apply for and maintain public benefits eligibility.
• Prepare detailed case documentation and reports to the court.
• Provide information concerning rights, options, benefits, services and goals.
• Conduct monthly visits and document findings.
• Collaborate with service and medical providers.
• Perform field visits to assess guardianship/health needs.
• Coordinate with community resources and partners for clients.
