Job summary

The Georgia Department of Human Services, Division of Aging Services, is seeking an Adult Guardianship Case Manager 2 in Marietta, Cobb County, with responsibility for additional counties. Travel throughout the area will be required as needed to support guardianship clients and families.

You will develop care plans, coordinate health and welfare services, and act as surrogate decision-maker, while educating clients, families, and providers in the multi-county region.

Qualifications

• Bachelor's degree from an accredited college or university (or equivalent experience).
• Strong communication and interpersonal skills are required.
• Ability to work with multi-county caseload and coordinate services.

Responsibilities

• Provides guardianship case management in accordance with law and policy.
• Develops care plans coordinating services for guardianship clients.
• Advocates for guardianship clients and coordinates with trustees or fiduciaries.
