Job summary

Georgia Department of Human Services (DHS) seeks an Adult Guardianship Case Manager 2 to provide guardianship services across Cobb, Clayton, DeKalb, Fayette, Fulton, Gwinnett, Henry, Rockdale and surrounding counties. Travel will be required as needed.

Responsibilities include coordinating care, advocating for clients, and documenting actions per policy. A bachelor’s degree and strong communication skills are required; telework options may be available depending on the position.

Qualifications

• Bachelor's degree from an accredited college or university required.
• Excellent oral, written, presentation, and interpersonal communication skills.
• Strong ability to document and coordinate case management actions.

Responsibilities

• Provides guardianship case management services in accordance with guardianship law and department policy.
• Develops care plans to arrange support, care, education, health and welfare for clients.
• Practices surrogate decision-making ensuring least restrictive options and client preferences.
• Coordinates with trustees, conservators and fiduciaries to meet financial needs.
• Provides information and education to guardianship clients and stakeholders in the multi-county area.
• Acts as mandated reporter of abuse, neglect, or exploitation as required by law.
