Job summary

Qualifications

Responsibilities

• Conduct in person visits to clients across service areas.
• Coordinate personal safety, problem solving and case management.
• Ensure timely medical and mental health services for wards.
• Attend medical appointments as needed.
• Maintain up to date needs assessments and goals for each ward.
• Monitor wards’ living situations and coordinate supports.
• Ensure guardianship court orders are followed and reports filed timely.
