Qualifications

• Licensed Physical Therapist in NY or eligible to practice
• Ability to evaluate a patient’s need for physical therapy and develop an appropriate treatment plan
• Provide care and treatment to patients based on physician care plan and professional standards
• Experience instructing family members, PTAs, Home Health Aides and other caregivers in appropriate physical therapy techniques
• Mandatory Certifications: First Aid, AED, CPR

Responsibilities

• Evaluate patient needs and develop personalized treatment plans.
• Deliver care and treatment per physician care plan and agency policy.
• Instruct caregivers and family members in proper therapy techniques.
• Maintain professional standards of care and documentation.
