Job summary

PureHealth is seeking a Certified Occupational Therapy Assistant to join our post-acute care team. Under the supervision of an Occupational Therapist, you will contribute to patient assessments and interventions while following PureHealth policies and state/federal regulations.

Responsibilities include documentation, treatment planning, and discharge coordination, with emphasis on quality care and teamwork.

Qualifications

• Graduate of an accredited COTA program.
• Minimum of one year of COTA experience.
• Valid state COTA license.
• Proficient in English with strong communication skills.
• Ability to work in a post-acute setting and rotate between locations.

Responsibilities

• Assist the Occupational Therapist with patient assessments and treatment planning.
• Provide patient-centered therapeutic interventions as directed.
• Complete documentation (charges, progress notes, reports) accurately and timely.
• Monitor treatment effectiveness and modify as needed.
• Educate patients and families about care plans and needs.
• Coordinate discharge planning with the health team and external providers.
• Maintain productivity and comply with policies and HIPAA requirements.
