Job summary

Axon Medical Centers in Longview, Texas is seeking a Physical Therapist (PT) to evaluate, diagnose, and treat musculoskeletal and injury-related conditions in an outpatient setting. The PT will collaborate with clinicians to create individualized plans and ensure well-documented, evidence-based care.

The ideal candidate will be clinically strong, patient-focused, organized, and eager to contribute to a growing rehabilitation program at our expanding Longview location.

Qualifications

• Doctor of Physical Therapy (DPT) or equivalent qualifying degree.
• Current Texas Physical Therapy license.
• Strong clinical assessment and treatment skills.
• Excellent communication and interpersonal skills.
• Strong documentation and organizational skills.
• Ability to work independently and as part of a multidisciplinary team.

Responsibilities

• Perform comprehensive physical therapy evaluations and examinations.
• Develop individualized treatment plans based on patient diagnoses, functional limitations, and goals.
• Provide evidence-based therapeutic interventions.
• Treat orthopedic, spine, musculoskeletal, and injury-related conditions.
• Monitor patient progress and modify treatment plans as clinically indicated.
• Document evaluations, daily treatment notes, progress reports, and discharge summaries accurately and timely.
• Maintain documentation in accordance with company, clinical, and regulatory requirements.
