The Therapist position provides quality psychotherapeutic and discharge planning services to all patient populations and their families; provides assessment, group therapy , family work, and discharge planning; serves as a member of interdisciplinary team supporting the organization’s treatment program and philosophy. This is a full-time, 40 hours / week position.

The Group Social Worker provides quality psychotherapeutic, group treatment, and discharge planning services to patients and their families. This position provides psychosocial assessments, facilitates therapeutic process groups, participates in treatment planning, provides family/collateral support, and coordinates discharge and aftercare services.

As a member of the interdisciplinary treatment team, the Group Social Worker supports the hospital's treatment philosophy and provides evidence-based, trauma-informed, and recovery-oriented care to patients.

Qualifications
  • Conducts and facilitates process group treatment and therapeutic group sessions for assigned patients.

  • Plans and delivers group interventions designed to support patients' treatment goals, coping skills, emotional regulation, recovery, and successful transition from inpatient care.

  • Performs and appropriately documents comprehensive psychosocial assessments, including mental, social, and physical factors and discharge planning needs.

  • Completes family and collateral contacts in conjunction with the psychosocial assessment and documents attempts and outcomes appropriately.

  • Participates in the development and implementation of individualized treatment plans by presenting relevant assessment findings and incorporating appropriate therapeutic interventions.

  • Develops realistic and measurable treatment goals and participates in the ongoing reassessment of patients.

  • Documents patient progress toward treatment, aftercare, and discharge planning goals.

  • Communicates patient progress and activities related to discharge and continuing care with patients, families, the treatment team, referral sources, and other individuals involved in the patient's care.

  • Ensures appropriate discharge arrangements are completed, schedules aftercare appointments, and assists patients and families in accessing community resources.

  • Coordinates and manages mental health commitment proceedings as appropriate.

  • Maintains professional relationships and appropriate boundaries with patients, family members, staff, and community partners.

  • Provides consultation, support, and modeling of evidence-based, trauma-informed, and recovery-oriented approaches to peers, trainees, MHTs, nursing staff, and other members of the treatment team.

  • Adheres to hospital and departmental policies, procedures, documentation standards, and regulatory requirements.

  • Participates in quality and performance improvement initiatives as they relate to Social Work services.

  • Performs other duties as assigned.

Master’s degree in Social Work, Clinical/Counseling Psychology or Social Services related field required. License not required but preferred. Able to provide LCSW supervision.

Experience

2 years of Clinical experience with prior experience working with a psychiatric mental health population preferred.
