Position Overview

Support Community Living Specialist (SCLS) – full‑time, Red Oak, Iowa. Hours: Monday/Tuesday 3 pm‑11 pm; every other weekend 9 am‑11 pm plus five hours of activities or appointments to meet full‑time weekly requirement.

Reporting

The SCLS reports directly to the Team Leader and the Director of Residential Services.

Responsibilities
  • Train, teach, and support individuals to develop skills for independent activities of daily living (ADLs).
  • Coordinate health and medical needs, including medication administration and diet planning.
  • Assist individuals in accessing community services and participating in at least one community activity per week (when scheduled during day/evening/weekend hours).
  • Advocate for individuals’ rights and interests; prepare individualized intervention plans.
  • Provide emergency and safety skill training, budgeting assistance, and transport to appointments.
  • Orient new individuals in their homes, assess skills, and provide input to Person‑Centered Plans.
  • Supervise goal attainment, collect data, and complete documentation according to agency procedures.
  • Maintain a therapeutic, supportive, and confidential environment; never leave individuals under 24‑hour care unsupervised.
  • Ensure cleanliness, safety, and health standards at individuals’ residences.
  • Attend staff meetings, training sessions, and keep informed of changes via email or daily logs.
  • Handle all written and verbal information about each individual with strict confidentiality.
  • Complete documentation of behaviors and contribute to staff intervention planning.
  • Assist with personal care tasks such as meal preparation, laundry, housekeeping, toileting, bathing, and overall personal dignity.
  • Monitor and document physician‑ordered diets and other medical instructions.
  • Actively participate in any additional duties assigned by the Team Leader, Lead Staff, or Director of Residential Services.
  • Adhere to agency policies, state and federal regulations (including HIPAA), and maintain accurate medication records.
  • Use sound judgment for immediate decision‑making and manage emergencies appropriately.
Essential Functions
  • Provide quality care and transportation as required.
  • Ensure home and community environments are safe, clean, and meet standards.
  • Work independently while safeguarding the welfare of residents.
  • Comply with HIPAA and protected health information (PHI) protocols.
  • Maintain cleanliness and safety for both residents and staff.
  • Support staff in training new team members as needed.
Requirements
  • At least 18 years old with a high‑school diploma or GED.
  • Prior experience teaching cooking, cleaning, and personal hygiene skills preferred.
  • Valid Iowa Chauffeur’s license with acceptable driving record.
  • Completion of pre‑employment physical, TB test, and drug screening; periodic testing every four years as required.
  • 10‑hour medication‑management class completed.
  • Physical capacity to transport up to 50 lb, prop up or transfer individuals, administer self‑care tasks, and perform routine household duties.
  • Willingness to handle biological fluids and maintain a clean work environment.
  • Demonstrated ability to work with clients exhibiting behavioral disorders, including aggressive behaviors.
  • Strong verbal and written communication skills; self‑motivated and team‑oriented.
  • No history of assault, abuse, or bodily injury convictions.
  • Participation in agency investigations and adherence to confidentiality policies.
Physical Requirements
  • Carry supplies up to 50 lb and travel up to 70 miles.
  • Enter and exit resident homes, including stair navigation with hands full.
  • Assist transfers from sitting to standing or prone positions, wheelchair handling, and shower/bath assistance.
  • Perform mandatory self‑care support and mandatory self‑care procedures.
  • Must not obligate activities that would compromise resident safety or personal dignity.
Security Clearance Levels for PHI Access
  • Level One: Minimum access – truck drivers, maintenance, volunteer staff.
  • Level Two: Direct care staff – SCLS, floaters, program instructors, Day‑Hab aids, vocational trainers, 1:1 staff.
  • Level Three: Office support staff.
  • Level Four: Team Leaders, supervisors, work center supervisors.
  • Level Five: Program managers.
  • Level Six: Directors/Administrators, Quality Assurance, IT staff.
  • Temporary or permanent clearances assigned as required for extended duties.
