DESCRIPTION

DUTIES & RESPONSIBILITIES
  • Work with individuals at intensive group homes that displays intense behavioral management.
  • Provide staff performance feedback, discipline, and document staff progress and performance. Report abuse and unethical behavior.
  • Communicate and participate in discussions, planning, and implementation of all operations within the group homes.
  • Complete weekly onsite visits on all shifts to ensure compliance and completion of job duties.
  • Ensure adequate staff to consumer ratio, quality programming, adequate coverage and overall maintenance of the homes.
  • Be available or find coverage during open work slots, emergencies, evacuations, etc.
  • Ensure medications are present in the home, communicating with the pharmacy, retrieving medications from the pharmacy, ensuring documentation of medication administrations and counts are accurate and ensuring the home is in in compliance based on 65.G7 (Medication Administration)
  • Provide leadership, support and supervision to group home staff, to include ongoing training and performance management.
  • Ensure that all supplies are on hand and repairs are completed in a timely manner in order to maintain quality care and services and conduct scheduled activities including program implementation, personal care, recreational activities, and community involvement.
  • Ensuring coordination, development, and implementation of each resident's individualized implementation plan.
  • Develop and monitor daily activity schedules and skill acquisition programs for each client.
  • Coordinate evaluation, admission and discharge of residents. Complete inventory and body checks for incoming and outgoing residents.
  • Coordinate activities of the consulting disciplines including physical therapy, occupational therapy, speech therapy, behavioral therapy, dietary services, medical/nursing and other interventions and services for each resident.
  • Ensure individualized implementation plans, program, activities, and related interventions/services are compliant with AHCA, any governing agency, the most recent Medicaid Waiver Handbook, Medicaid Handbook and/or other federal/state regulations.
  • Complete and distribute required reports/paperwork on or before the due date, including but not limited to annual reports, Individual implementation plans, quarterly reports, and active treatment plans.
  • Ensure optimal communication and integration of individual programs and activities with ADT, schools, and any outside service, training/development an individual receives.
  • Coordinate the purchase, use, inventory, and maintenance of each assigned individual's belongings as well as activity and therapeutic equipment and supplies.
  • Coordinate with outside agencies in order to provide integrated services to the individuals served. Participate in meetings with outside agencies as needed.
  • Conduct onsite visits to behavioral homes to ensure compliance with behavioral principles and correct implantation of Behavior Plans.
  • Conducts auditing of client files to ensure compliance. Follow up with and correct errors found during audits.
  • Establish a positive relationship with parents and guardians through written and verbal communication and coordinate home visits.
  • Plan and coordinate outings that meet the preferences of the clients and encourage community inclusion.
  • Maintain and monitor financial records for all clients.
  • Abide by the Medicaid Waiver Handbook and APD (Agency for Persons with Disabilities) regulations
  • Complete all other duties as assigned and.
