Family Health Center of Worcester is a Federally Qualified Community Health Center that provides care for more than 30,000 patients annually in over 80 languages.

Program Specific Essential Duties and Responsibilities:

Portfolio Strategy and Opportunity Qualification:

  • Manage an assigned portfolio of approximately 15-20 federal, state, and private foundation grants and selected contracts across the full lifecycle, from prospect identification and proposal development through closeout.
  • Identify, research, and evaluate prospective funding opportunities from government agencies, foundations, corporations, and other institutional sources, prioritizing opportunities that align with organizational goals and offer meaningful impact.
  • Participate in grant strategy with members of Development Team and Executive Leadership Team, and facilitate internal vetting of prospective opportunities, providing recommendations regarding strategic fit, readiness, resource requirements, compliance obligations, and sustainability.
  • Manage the “Award Research and Qualification” process to identify new funding opportunities, streamline how opportunities are vetted, and intake funding needs for strategic, clinical, and other programmatic needs.

Proposal Development and Funder Cultivation:

  • Lead the development of clear, compelling, and persuasive proposals, letters of inquiry, and related materials for assigned portfolio.
  • Develop and manage proposal development workplans to ensure complete, accurate, and timely submissions, including coordinating contributions from other staff members. Collaborate with Finance and program teams to develop grant budgets, budget narratives, staffing plans, sustainability plans, and other financial or operational components of applications.
  • Gather and analyze relevant, reputable research, program data, outcomes, needs assessments, case statements, and supporting documentation to incorporate into proposals and related materials.
  • Ensure that all applications meet funder requirements, industry standards, and applicable compliance requirements before timely submission.
  • Build and maintain productive relationships with prospective and established funders, program officers, and other external partners, serving as a primary or delegated point of contact for assigned opportunities and awards.

Award Launch, Administration, and Compliance:

  • Coordinate post-award review and launch for assigned grants and contracts, interpreting funder terms, approved budgets, deliverables, staffing commitments, and reporting requirements into clear operational guidance and internal workplans.
  • Maintain regular communication with program and administrative partners to monitor program implementation, identify risks or delays, and support timely resolution of grant-related issues.
  • Lead the preparation and submission of required programmatic reports, ensuring alignment among narrative results, performance data, budgets, staffing, and funder requirements
  • In collaboration with programmatic grant managers, prepare for and participate in funder site visits, monitoring activities, and audits, and provide project management support for follow-up actions.
  • Coordinate contract, report, application, and other grant documents for internal review, signature, and submission by required deadlines.

Financial Management and Reporting:

  • Collaborate closely with Finance and program leadership to monitor grant budgets, expenditures, projections, and fiscal performance for assigned awards.
  • Coordinate spend-down strategies, budget amendments, and multi-year budget planning to support timely, accurate, and compliant use of funds.
  • Manage the “Grant-Funded Staffing” process to ensure alignment between accounting and payroll records.
  • Advise program managers and organizational leadership regarding the budget, staffing, compliance, and operational implications of awards, renewals, amendments, and expirations.

  • Maintain complete, accurate, and current records for assigned prospects, submissions, awards, contracts, funder communications, deadlines, reports, and related documents in departmental databases (Bloomerang) and other tracking systems.
  • Contribute to the design, documentation, and continuous improvement of grant management systems that strengthen coordination, efficiency, compliance, and organizational risk management.
