Total Care Therapy in Granger, IN is seeking an Occupational Therapist to evaluate residents and develop individualized treatment plans to improve independence in ADLs and cognition in assisted living and independent living communities.

Qualifications

• Current State OT license.
• CPR/BLS certification.
• Valid driver's license and reliable transportation.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and establish individualized OT plans of care.
• Provide skilled OT interventions to improve ADLs, IADLs, upper extremity function, cognition, balance, coordination, and functional independence.
• Assess and recommend adaptive equipment and environmental modifications.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the EMR.
• Educate residents, caregivers, and facility staff on adaptive techniques, home safety, fall prevention, and energy conservation strategies.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
