Job summary

Total Care Therapy LLC in Schererville, IN is seeking a dedicated Occupational Therapist for a full-time role in assisted living and independent living communities. The OT will evaluate residents, develop individualized treatment plans to improve ADLs, cognition, and safety, and collaborate with an interdisciplinary team.

Responsibilities include supervising COTAs, documenting in EMR, and educating residents and staff on safety and energy conservation.

Qualifications

• Current State Occupational Therapist (OT) License is required.
• CPR/BLS Certification is required.
• Valid driver's license with reliable transportation is required.

Responsibilities

• Evaluate residents and establish individualized OT plans of care.
• Provide OT interventions to improve ADLs, IADLs, upper extremity function, cognition, balance and safety.
• Assess and recommend adaptive equipment and environmental modifications.
• Reassess progress and modify treatment plans as needed.
• Supervise COTAs in accordance with state regulations.
• Document care using EMR.
• Educate residents, caregivers, and staff on home safety and energy conservation.
• Collaborate with interdisciplinary team for coordinated patient-centered care.
